Gluten challenge should be performed in patients who embarked on a gluten- free diet empirically without serologic or pathologic abnormalities. It can also be done in patients with latent or potential celiac disease. Gluten should be started in small increments beginning with one slice of bread and doubling this amount every 3 days if tolerated until at least 1.
